How to Make a Form QR Code in 3 Steps

Pick the Form Type and a Template

Choose Form in the generator above, then start from one of eight templates or a blank form. Templates are a starting point: every field can be renamed, reordered or removed.

Add Your Fields

Up to 30 fields from nine types. Mark the required ones, add placeholders, write the title, consent text and the confirmation message. The phone preview beside the builder shows exactly what a scanner will see.

Every field takes a label, a placeholder and a required toggle. Choice fields hold up to 20 options. A hidden field is never shown to the scanner but filled from the scan link, so one form printed in three places can record which place each submission came from.

What the Scanner Sees, and Where It Lands

A Mobile Form, Nothing to Install

The code opens a hosted form sized for a phone: your title and description, the fields in your order, the consent text and your submit button. No app, no login, no account for the person filling it in.

Validated on the Server

Required fields must be filled, email, phone and number fields are checked for format, and a redirect after submit only ever goes to a web address. Bad entries are rejected before they reach your list.

Your Confirmation, Your Next Step

Write the message people see after submitting, or send them on to a page of yours: a calendar, a download, a thank-you page with directions.

Submissions Next to the Scans

Every submission is listed on the QR code's analytics page, newest first, beside the scan count for the same code. Scans minus submissions is your drop-off.

CSV That Opens Cleanly

Export the list as a CSV for Excel or Google Sheets. Multiple checkbox answers are joined in one cell, and values that look like spreadsheet formulas are neutralised so a submission can never run code in your sheet.

Know When Someone Submits

Turn on the email notification and each new submission lands in your inbox. On Pro and above, submissions with an email field can also sync to HubSpot as contacts.
