How to Find Your Google Form Shareable Link
Before you can create a QR code, you need your Google Form's shareable link. Google Forms doesn't have a built-in QR code option, but getting the link takes 30 seconds:
Open Your Form
Go to forms.google.com and open the form you want to share. Make sure you're in edit mode (not preview).
Click “Send”
Click the purple Send button in the top-right corner. This opens the sharing options dialog.
Click the Link Icon
In the Send dialog, click the link icon (the second tab — it looks like a chain). This shows your form's shareable URL.
Copy the Link
Optionally check “Shorten URL” to get a shorter link. Then click Copy. Your Google Form URL is now on your clipboard.

Come back to the generator above and paste your link. Click “Generate QR Code” — done.
Pro tip: Make sure your form's sharing settings are correct. Under “Send,” check that “Collect email addresses” and “Limit to 1 response” are configured the way you want before generating the QR code.
Three steps
Create Your Google Form QR Code in 3 Steps
Paste Your Google Form Link
Step 1Copy the shareable URL from your Google Form (see instructions above) and paste it into the generator. UseQRKit accepts any Google Form URL — surveys, quizzes, registration forms, feedback forms, order forms, and more.
Customize Your QR Code
Step 2Choose colors, add your logo or brand mark, and pick a style that matches your materials. A branded, visually distinct QR code gets scanned more often than a generic black-and-white square.
Download and Share
Step 3Download your QR code as PNG (for digital use) or SVG (for print). Add it to posters, flyers, table tents, presentations, email signatures, or websites. Because it's a dynamic QR code, you can change the linked form anytime from your dashboard.
Overview
What Is a Google Form QR Code?
A Google Form QR code is a scannable code that links directly to a Google Form. When someone scans the code with their phone camera, the form opens instantly in their mobile browser — ready to fill out. No app download, no typing a URL, no searching for a link.
Google Forms is used by over 47% of the online form market for surveys, quizzes, registrations, feedback collection, and data gathering. But sharing a form link by text, email, or chat creates friction. People forget to open the link, lose the message, or don't bother typing a URL they see on a poster or presentation slide. A QR code removes that friction and can boost form response rates significantly.
That's why organizations from universities to restaurants to HR departments use QR codes to get more form submissions. Scan, fill out, submit — done in seconds. You can create one with any QR code generator that supports URL input.
Does Google Forms have a built-in QR code feature? No. Google Forms only lets you share via email, link, or embed code. To create a QR code for your form, you need a third-party generator like UseQRKit. A Google Form QR code is essentially a URL QR code — you copy the form's shareable link and paste it into the generator. It takes about 10 seconds.
Use cases
Where to Use Google Form QR Codes
Event Registration & Check-In
Print QR codes on event posters, invitations, or banners. Attendees scan to register or check in. Use a Google Form with name, email, and ticket type fields. Track registrations in real time via the linked Google Sheet.
Classroom Attendance
Post a QR code on the classroom screen or door. Students scan and submit their name and student ID to mark attendance. Teachers get instant records in Google Sheets without manual roll calls.
Customer Feedback & Surveys
Place QR codes on receipts, table tents, product packaging, or at checkout counters. Customers scan and rate their experience while it's fresh. For Google-specific reviews, try a Google review QR code.
Employee Feedback & HR Surveys
Post QR codes in break rooms, on bulletin boards, or in internal newsletters. Employees scan to submit anonymous feedback, safety reports, or pulse surveys. No need to track down email addresses or send calendar invites.
Lead Capture at Trade Shows
Display a QR code at your booth or on handouts. Prospects scan to fill out a contact form, request a demo, or download a resource. Responses flow directly into a Google Sheet you can import into your CRM.
Restaurant & Hospitality Feedback
Place QR codes on table tents, menus, or near the exit. Guests scan to rate their experience, flag issues, or leave suggestions. Pair with a WiFi QR code for easy guest connectivity.
Product Feedback & Reviews
Include a QR code on product packaging or inserts. Buyers scan to share their experience, report defects, or request support. The form captures structured feedback (star rating, comments, photos) that you can analyze in Sheets.
School & University Applications
Share QR codes at college fairs, on brochures, or on campus signage. Prospective students scan to submit interest forms, campus visit requests, or scholarship applications.
Compare
Static vs Dynamic — Which QR Code Do You Need?
It depends on whether your form will change over time and whether you want to track scans.
Static Google Form QR Code
FreeEncodes the Google Form link directly in the QR pattern. Works forever, no account needed. But if you update the form or close it, the QR code still points to the old version. Cannot be tracked or changed after printing.
Best for
One-time forms, permanent surveys, simple handouts.
Dynamic Google Form QR Code
RecommendedUses a short redirect URL you control from your dashboard. Swap the form URL anytime, track scans (when, where, what device), and deactivate if needed — all without reprinting.
Best for
Recurring events, semester surveys, multi-location setups, anything that might change.
Our recommendation: Use a Dynamic QR Code for any form that might change — new semester surveys, updated event registrations, or rotating feedback forms. The ability to update the linked form without reprinting is invaluable.

Tips for Getting More Responses With Google Form QR Codes
Add a Clear Call-to-Action
Don't print a bare QR code. Add text like “Scan to share feedback,” “Scan to register,” or “Scan to take the survey.” People are more likely to scan when they know what to expect.
Keep Your Form Short
Mobile users filling out a form on their phone will abandon long surveys. Aim for 5–10 questions. Use multiple choice, rating scales, and dropdowns instead of open text fields.
Test on Multiple Devices
Scan the QR code from both iPhone and Android before printing. Make sure the form loads correctly and fields are easy to tap. Use our QR code scanner to verify.
Use the Right QR Code Size
For table tents and handouts (scanning from ~30cm), print at least 2cm × 2cm. For posters (scanning from 1+ meter), print at least 10cm × 10cm. A QR code that's too small to scan defeats the purpose.
Enable Form Settings Correctly
Before generating the QR code, check: Is the form accepting responses? Is “Limit to 1 response” on or off? Is email collection enabled? These settings affect the user experience after scanning.
Consider Pre-filling Form Fields
Google Forms supports URL parameters to pre-fill answers. Add parameters to your form URL before pasting it into UseQRKit to pre-fill event names, location identifiers, or source tracking. Share documents alongside forms with a PDF QR code.
